What you need
A high-quality PDF scan of a form, or the form in .docx or .xlsx format, that’s no more than 1MB. The form should contain title text for fields needed in the form, prompting the user to input data in the digital version.
Where to Convert
On the screens page of your account, instead of adding a new form, hit “Convert Document” to pop-up options to convert your document into a digital form.
If the uploaded form isn’t blank and has data already filled in, i.e., populated answer values, these values can be set as hint text for the fields. And, if the form contains multiple pages, you can replicate this in the converted design or keep everything on a single digital page.
Expectations (beta)
In its current state, you can expect the basics from singular data capture fields to yes/no Choices field options and repeatable data capture tables.
Outputs might not hit the mark entirely depending on form complexity, but they provide a starting point instead of starting from scratch.
